Update Your Sports Physical!

All tryout participants must have an updated physical dated after March 1, 2021. Sports physicals are valid for 365 days. Physicals must be completed by a Primary Physician, or within eligible medical offices (urgent cares, etc). To attain a printable copy of the sports physical, click HERE.
